Interim Practice Manager/Patient Services Manager

Castle meadows surgery now has a unique service to improve access for patients to general practice services and help with nursing and care needs whilst giving patients the continuity of care by dealing with one named individual at the practice.

If you need to know how to get test results, need to know what’s happened to your referral or you want to know how to get a wheelchair this is the person you would contact at the surgery. These are just examples, but by having a named individual you can contact you can be assured that your problem will be dealt with and you can turn to the same individual along the way to find out how your problem is being resolved.

The Patient Services Manager also acts as care co-ordinator for the most frail patients with complex needs. For each such patient a care plan is devised by a GP lead at the practice in conjunction with the patient and/or their carers. The management of that plan is co-ordinated by the Patient Services Manager.
