Our practice can provide the following services: ​

• Well woman and well men checks
• Smears
• Minor surgery
• Holiday and routine immunisations – If you have been advised by the surgery to complete a travel risk assessment, you will need to complete our Travel Risk Assessment Form.
• Chronic disease management (diabetes, chest disease, vascular and heart
  disease)
• Blood tests –  If you would like to request your test results, please use our test results triage.
• Urine testing for infection and pregnancy
• Counselling service
• Corticosteroid Injection Consent – If you need to provide consent for corticosteroid injection, please submit a Corticosteroid Injection Consent form.
